§ 478.21 Forms prescribed.

  1. a.(a) The Director is authorized to prescribe all forms required by this part. All of the information called for in each form shall be furnished as indicated by the headings on the form and the instructions on or pertaining to the form. In addition, information called for in each form shall be furnished as required by this part. The term ‘sex’ on ATF forms required by this part refers to an individual’s immutable biological classification as either male or female and does not include the concept of gender identity. Individuals completing forms required by this part should select their biological sex. Each form must be executed under penalties of perjury, if the form or the regulation so provide.

  2. b.(b) Requests for forms should be submitted to the ATF Distribution Center ( http://www.atf.gov ) or made by calling (202) 648-6420.
